PHC Partnership Coordinator VISTA
 
 
 
The Upper Valley Region served by the Public Health Council (PHC) includes 12 towns in New Hampshire and 22 towns in neighboring Vermont. The population of this region is approximately 90,000 with 52% of those with incomes less than $24,999. The PHC works with community leaders and representatives from a diverse group of community sectors. Partners work together to create a healthier, safe, supportive, and vital Upper Valley. Residents and stakeholders identified a host of significant population health concerns: access to mental health care; heroin and misuse of pain medications; alcohol and drug misuse; access to dental health care and access to affordable health insurance and prescription drugs. Residents see value in addressing the social determinants of health. These include access to affordable housing; affordable, high quality child care; access to healthy, affordable food; substance abuse recovery systems and public transportation. Stakeholders identify groups who are “left behind.” These include: people in need of mental health care; low-income populations; people in need of substance abuse treatment, the under and/or uninsured; homeless and seniors. The PHC encourages collaboration among organizations as a means to deliver core public health services, establish public health priorities, and mobilize resources to achieve measurable outcomes. The proposed project will help the PHC build capacity to serve the needs of towns and organizations in the 22 Vermont communities recently added to our region. The project will strengthen our capacity to reduce health disparities and address the social determinants of health across our entire region.

Member Duties : PHC proposes to sponsor an AmeriCorps VISTA to serve as a Partnership Coordinator. The VISTA Member will assist in developing an outreach and fundraising plan to build capacity in low-income Vermont communities; manage communications with and between the working groups of the Public Health Council; and promoting the work of the Public Health Council in the region. The VISTA Member will allow the PHC to build additional communications and outreach capacity following a major expansion in the number of towns in its region. While we have relationships with many VT-based organizations and some municipalities, we need to take a more systematic approach to introducing ourselves to these towns. This Partnership Coordinator VISTA Member will also assist the Executive Director by providing support to several of the PHC's working groups and general communications. The VISTA Member will assist with data collection and reporting systems.
