2017 PHMC Shelter-Based Nursing Program
 
 
 
Families experiencing homelessness have complex medical and behavioral health needs. Many of these families are affected by trauma. Families with young children in emergency shelters experience an elevated number of adverse childhood experiences (ACEs), with higher levels of ACEs among parents predicting more child adversity. Many families experiencing homelessness have also experienced ongoing, interpersonal trauma, including community and domestic violence and physical, emotional, and sexual abuse, and homelessness itself can be a traumatic experience. Repeated traumatic events can serve as a barrier for families to participate in services and supports and form trusting relationships, and present serious risks to children’s and parents’ functioning and well being. Adversity also places homeless children at risk for multiple health problems, increased hospitalizations/ER visits, malnutrition, immunization delays, elevated lead levels, asthma and infectious disease. The VISTA will work with the Shelter-Based Nursing program to educate these families, improve the data system and increase accurate collection of data. The VISTA will also identifying grant opportunities to supplement the goals of the program.

Member Duties : 1. Researching and develop curricula & educational materials for the Healthy Baby Initiative. 2. Researching and develop curricula to help homeless mothers have a better understanding of pediatric development, risks, interventions & importance of primary care. 3. Preparing educational materials on chronic disease. Develop and implement group activities and individual training that support a healthy lifestyle for shelter residents. 4. Researching and develop community health related resources to enhance life-long health awareness and participation. Develop partnership with community-based resources (i.e., gym memberships, etc.) that initiate life style changes. 5. Developing outcome data gathering systems to implement data management to maximize measurement of program outcomes. 6. Identifying and process grant applications which can supplement the goals of the program and incentivize participation (ex. incentives for HIV testing, participation in Healthy Baby Initiative).
