Community Action Akron Summit Community VISTA 2021
 
 
 
The Pathways Community HUB is a network of agencies housed under Community Action Akron Summit who is the fiscal and administrative agent. The purpose of the Hub is to eliminate infant mortality and reduce the burden of chronic disease in Summit County. The work at the HUB is to provide training and technical assistance to the sub-contracted agencies, manage and coordinate referrals and enrollments, bill Medicaid for services rendered and conduct outreach and educational campaigns. The VISTA member’s project would be to develop training sessions and community outreach campaigns as well as contributing to grant proposals and increasing the referral and outreach partnerships. The HUB does not provide direct services, rather it provides the infrastructure for the sub-contracting agencies to provide direct care. The Hub also builds partnerships and resources to support the on-going needs of the families in the HUB. These include identifying workforce development opportunities, secondary education and housing services. The role of the VISTA member would be to identify new resources to support the pathways and needs of the families enrolled as well as training resources. The VISTA member will research and develop COVID -19 education to support a community-wide campaign to promote vaccine uptake and prevention. All of this work will support the goal to improve health, social and economic outcomes of the families enrolled in the HUB.

Member Duties : Under general direction, serve to build referral partnerships for the network of community health workers; research and identify credible education materials about Covid vaccine and prevention; organize an outreach campaign about Covid vaccine and prevention strategies; assist in grant writing and identifying community resources for the network of HUB partners; assist in developing outreach and publicity campaign about the HUB services and network; research and identify possible training opportunities for CHWs and HUB staff; support data collection and data entry for customer service calls.
