ORGANIZATION: Sabathani Community Center, founded in 1966, is a landmark social service site in South Minneapolis. Location, convenience, and the breadth of service offering allow our health program to provide for neighborhood residents who are limited to access preventative health services and other medical services due to limited income and poor access to transportation. We serve 32,000 residents each year. CHALLENGE AND PROGRAM INNOVATION: Sabathani Community Center’s Senior Program is one of the oldest culturally-relevant senior program models in the Twin Cities. The Senior Program existed even before Sabathani Community Center was officially recognized as an organization. Sabathani’s Senior Health Outreach Program is are most recent accomplishment. We launched the program in November, 2015 and by 2016, Sabathani’s Health Outreach program was recognized as a best practice program to create an equitable system for aging adult services. GOALS: Our aim for the project is to improve health outcomes for Somali, Latino, and African American Minnesotans in South Minneapolis, through expansion of our outreach services and coordination with nurse training programs. ---This project is part of the College Health Corps (CHC) VISTA Program, operated by Minnesota Campus Compact. The CHC increases low-income individuals’ access to health care, health education, food, and basic needs through capacity building projects, including partnerships between higher education and community based organizations. Professional development focuses on health equity, the social determinants of health, and effective partnership development. --- ***Interviews will occur on a rolling basis. Member Duties : The AmeriCorps member will enhance our current outreach program to increase health access for our service population of 32,000 annual clients (28,000 of whom are low-income communities of color). There are three main roles we envision for the AmeriCorps member. 1. Augment existing infrastructure of long-term community partners, including service sites and nursing programs, in order to expand our network of free/low-cost health providers. 2. Design training for volunteers to perform outreach, so that all outreach nursing students receive identical training in community health work. 3. Create an innovative, culturally-competent, needs assessment, survey, and data tracking system to define client health needs. The result of the coordination will be expansion of free diabetes/hypertension prevention services at 12 relevant community sites.
