CAN Clinic Outreach Coordinator Summer Associate VISTA
 
 
 
The CAN Clinic has received 2 grants that must be worked together. Each program will involve working with our current medical population and recruiting new participants. These programs will need the expertise of Summer Associates that are motivated to develop new methods of working with a Hispanic population that are not accustomed to navigating a Health & Wellness system. The Summer Associate must be fluent in English & Spanish including the written language. Both programs will be working with people of poverty who do not have accessibility to medical or health & wellness programs. The goal is to introduce a new way of thinking for this population. The program aims to educate patients and their families how they can take control of their health and change the course in wellness. Under the supervision of the Community Health Worker or Nurse, the Summer Associate will have direct contact with current patients and to recruit new participants. These programs are focused on nutrition and exercise. The Summer Associate may be doing one on one teaching on such areas as diabetes or high blood pressure or leading an exercise class in our gym. The Supervisor will train the Summer Associate in these areas to enhance their own learning of the material. The Summer Associate will also be developing surveys to assess the participants learning. It is important that the Summer Associate be comfortable in meeting new community leaders, networking in new groups and delivering group and individual teaching to the participants. The Summer Associate will receive hands on training by their supervisors. This will be a team effort of the Community Health Worker, Nurse and two Summer Associates. We are confident the Summer Associates will receive great experience and satisfaction in assisting Cornerstone to change the course of wellness in a vulnerable population.

Member Duties : • Interview patients • Develop and implement surveys • Attend network groups in the community • Data entry of medical metrics • Follow up with participants to determine their participation and success of the program • Develop and implement education classes for the patient and family • Facilitate outside groups who are delivering educational programs • Work closely with Community Health Worker on establishing successful goals for participants
