SERVE Philadelphia Healthy Futures Coordinator
 
 
 
The Office of the Deputy Managing Director for Health & Human Services is currently looking for a Healthy Futures Coordinator to help drive efforts to improve access to vital benefits and services that help to stabilize health among low income populations, with a specific focus on immigrants and refugees, seniors, working families, ex-offenders, and adults aging out of the foster care system. This includes supporting citywide efforts to increase enrollment in health insurance through the Affordable Care Act and other public benefits. As part of this work, the Healthy Futures Coordinator will help to advance the work of Shared Prosperity Philadelphia – a citywide anti-poverty plan – by coordinating the efforts of the Shared Prosperity Benefits Access Working Group, which is developing strategies to leverage existing resources to increase participation in key benefits (such as health insurance) and supportive programs (such as asthma remediation programs) to promote healthy lifestyles. The Healthy Futures Coordinator would also manage the Children and Families cabinet. The Children and Families Cabinet brings together key City agencies – including Human Services, Behavioral Health, Health and Supportive Housing – to identify ways to better coordinate efforts to promote health and family stability. This VISTA project will help to develop a common agenda for our partners, develop new pilot programs to increase health insurance enrollment, and develop evaluation tools and metrics to measure our progress.

Member Duties : Develop agendas and materials for monthly housing working group meetings. Develop a data dashboard to track activities. Develop tools to share information across partners and with the public. Coordinate the implementation of pilot projects that will later be brought to scale. Develop and implement a work plan to increase health insurance and other benefits among exiting members of the prison population. Convene best practices for data development and research. Develop trainings and information sessions. Identify and recruit partners.
