SerVermont - Senior Solutions

Senior Solutions provides services to older adults in their homes and local communities that help them remain independent, prevent impoverishment and achieve an optimum quality of life. We are a designated Area Agency on Aging through the Older Americans Act, serving our region since 1973. Services are targeted to those with the greatest economic and social need. Services include: HELPLINE offers information, referrals and assistance to older adults and caregivers to problem-solve, plan for the future and locate community resources. Assistance is provided with Medicare and public benefits such as food stamps and fuel assistance, and the “Special Help Fund,” supported by donations, helps address needs not covered by other programs. Community Outreach – We use multiple forms of media and build relationships in our communities to get the word out about available programs and services. We help people meet their needs and solve problems. Volunteer Programs – We recruit, train and support volunteers of all ages to assist isolated older adults. Case management – We work with an elder in their home to create and monitor a plan of care, centered on the elder’s personal values and preferences, that puts into place a variety of services. Family caregiver support – We help caregivers assess their family’s needs and options, connect with resources and provide short-term relief. Nutrition services and programs – We administer the “Meals on Wheels” and congregate meals programs in our region, and assures that all nutrition, food safety and other legal and administrative requirements are met. Wellness Programs - We provide a variety of evidence-based wellness and falls prevention programs including Tai Chi, aquatics, screening for medication related problems, and PEARLS for late-life depression. Other Services – Transportation programs, elder abuse assistance, legal assistance, assistance for adults with disabilities and home-based mental health services.
