SerVermont - Senior Solutions
 
 
 
Senior Solutions is a Vermont Area Agency on Aging that provides services and supports to older adults and caregivers in their homes and local communities that help them remain as independent as possible, prevent impoverishment and achieve an optimum quality of life. Services are targeted to those with the greatest economic and social need. Services include: • Information, referral and assistance - SENIOR HELPLINE offers information, referrals and assistance to seniors, their families and caregivers to problem-solve, plan for the future and locate community resources. Assistance is also provided with Medicare and public benefits such as food stamps and fuel assistance, and thea “Special Help Fund,” supported by community donations, helps address needs not covered by other programs. - Options Counseling provides assistance to individuals in understanding their options for long-term services and supports and making informed decisions. • Case management – Senior Solutions works with an elder in their home to create and monitor a plan of care, centered on the elder’s personal values and preferences, that puts into place a variety of services. Many clients would not be able to remain in their homes but for the services of Senior Solutions. • Family caregiver support – Senior Solutions can help caregivers assess their family’s needs and options, connect with resources and provide short-term relief. • Nutrition services and programs – Senior Solutions administers the “Meals on Wheels” and congregate meals programs in our region, and assures that all nutrition, food safety and other legal and administrative requirements are met. • Transportation - Senior Solutions supports transportation services for seniors that may include a van, a taxi, or a volunteer driver. • Other Services – Senior Solutions supports health, wellness and fall prevention programs, legal assistance, assistance for adults with disabilities and home-based mental health services.

Member Duties : The VISTA will help alleviate poverty by empowering people through knowledge of available resources and strengthening families and communities to better support those at risk. The VISTA will help develop and disseminate creative messages using multiple forms of media to provide information for and about older adults and aging issues. The VISTA will also help us expand our impact by finding and pursuing new funding sources. The VISTA will help build a volunteer program, create defined roles for volunteers and assure training, support and supervision. Our nutrition programs play an instrumental role in reducing food insecurity. The VISTA will strengthen local nutrition programs and expand their impact through volunteer recruitment and support. He or she will also recruit volunteers to serve as friendly visitors to homebound elders, and create volunteer opportunities for older adults. Volunteers will expand our capacity to disseminate information, answer questions and make referrals.
