Yamhill Community Care – Public Health Community Engagement
 
 
 
This position joins the Oregon Health Authority’s statewide VISTA team. In addition to activities at your individual site, each member receives additional orientation, training throughout the year, team retreats and meetings, and the support of the team leaders and project director based at the state public health agency. For more information about the Oregon Health Authority VISTA Partnership Program, please visit: http://go.usa.gov/39Vd5 Yamhill Community Care Organization (CCO) manages care and provides resources for members on the Oregon Health Plan, Oregon’s Medicaid system. Oregon Health Plan assigns Medicaid members to a CCO, which offers members additional support, including a designated primary care provider, services to manage chronic conditions, and other benefits designed to address the whole person and reduce barriers. While there is a great deal of support from community members and organizations, often there is not capacity to develop, manage, and provide all the services and tasks needed. The VISTA members will ensure that Yamhill CCO can achieve as many of its goals as possible, much sooner than would otherwise be feasible. Yamhill CCO, while small, is rapidly growing, and the programs the VISTA supports will contribute to an ongoing mission of improving the quality of life and health for those in poverty. The VISTA offers Yamhill CCO the ability to invest time in building a strong foundation for the organization, which enables future work to be better, more streamlined, and more effective. The VISTA will be one of two VISTAs serving at this site in McMinnville, Oregon, with two others in nearby partner organizations. Nestled in the heart of wine country, McMinnville is home to Linfield College and Evergreen Aviation & Space Museum. From here it is just an hour to Portland, an hour to Oregon’s spectacular coast, and just a bit further to Mount Hood, all surrounded by gorgeous hiking and sightseeing opportunities.

Member Duties : The VISTA will address barriers regarding access to health information, making it easier for patients, especially those at-risk or in poverty, to take charge of their own care. The VISTA will coordinate and develop education programs and materials for new and existing Oregon Health Plan (Medicaid) members, designed to improve understanding of their benefits and increase utilization of care. This will include organizing the launch of health education classes, wellness workshops like tai chi and self-management, and creating educational materials. The VISTA will also coordinate educational programs for healthcare providers, designed to create better communication and accountability between doctor and patient. The VISTA will work to improve outreach and engagement efforts by building connections with organizations that address vulnerable populations and maintaining our online communications. Car recommended; travel meetings and events reimbursed.
