Polk County Public Health – Community Council & Assessment
 
 
 
This position joins the Oregon Health Authority’s statewide VISTA team. In addition to activities at your individual site, each member receives additional orientation, training throughout the year, team retreats and meetings, and the support of the team leaders and project director based at the state public health agency. For more information about the Oregon Health Authority VISTA Partnership Program, please visit: http://go.usa.gov/39Vd5 This program is at the acutely important intersection between healthcare and public health. Willamette Valley Community Health (WVCH) is a Coordinated Care Organization that serves about 93,000 Medicaid recipients in Marion and Polk Counties, many of whom are minority and rural members that WVCH has struggled to engage and empower on their Community Advisory Council (CAC). As a Coordinated Care Organization, WVCH is required to operate a Council made up of at least 51% Medicaid consumer members. The Council oversees the development of the official Community Health Assessment and Improvement Plan. The VISTA will spend half of their time focused on building capacity for the Council and the other half of their time focused on the completion of the Polk County Health Assessment and Improvement Plan. The health information stemming from the findings of the assessment will be presented at community forums, in addition to trainings on providing culturally competent care. The VISTA will work to build the infrastructure of the Council, and forge relationships with community-based organizations in order to complete the Health Assessment and Improvement Plan. The target population is primarily those facing significant barriers to care, low-income families, minority, and rural populations. The VISTA service site will be the Polk County Public Health Department located in Dallas, Oregon. Centrally located, Dallas is 15 miles west of Salem, the State Capitol, about an hour from Portland, the Pacific Ocean, and Mt. Hood.

Member Duties : Working with a Coordinated Care Organization and Public Health partners, the VISTA will impact health disparities through capacity building activities and assessing the health of Polk and Marion Counties. The VISTA will work with a team in an office setting and out in the community, receiving hands on experience in developing a comprehensive community health assessment and improvement plan. This intersection of public health and health care will drive an innovative approach to culturally competent practice in the Mid-Willamette Valley. The VISTA member will work as part of a team with the Coordinated Care Organization serving Medicaid members, Willamette Valley Community Health and public health partners to build, implement, and evaluate targeted recruitment and orientation strategies for the Community Advisory Council. The other major project will be the completion of the Community Health Assessment and Improvement Plan which will guide public health and healthcare reform for years.
