Northern Colorado Health Alliance
 
 
 
The goal of the North Colorado Health Alliance is a population of healthy individuals who choose healthy behaviors, live in healthy communities, and who have 100% access to high quality and affordable care. The North Colorado Health Alliance is coordinating Community Care Corps project, to identify and address social determinants in Weld County Colorado. Community Care Corps workers identify and assess non-medical factors contributing to health neighborhood-by-neighborhood; synchronize delivery of campaign health messaging within their neighborhoods; connect residents with existing resources; and engage the public in overcoming obstacles to good health and prosperity. The Community Care Corps outreach team works to assist individuals, schools, neighborhoods, and communities in mobilizing and strengthening personal and social assets that can be brought to bear in generating health and reducing chronic disease. In 2010, an estimated 35,454 (14.3%) of the total population in Weld County (252,825) were living at or below poverty level. Weld’s poverty level is slightly lower than that of the United States as a whole (15.3%) and slightly higher than that of Colorado (13.2%). The percent of Weld County children who live in a family with a total income below the poverty level is 18.5%. Weld County has a lower average income than the rest of Colorado, and over one-fourth (27%) of households live on incomes that are below 200% of the federal poverty level. The North Colorado Health Alliance was incorporated as a 501(c)(3) non-profit organization in 2002. The Alliance, based in the town of Evans in Colorado’s Weld County, is a creative and strategic collaboration of partner organizations that are dedicated to cultivating the health of the communities they serve.

Member Duties : The primary focus of the Community Care Corps project is to address barriers to health by identifying social determinants of health in Weld County. The VISTA will work on program development and outcomes measurement for the project focus in the schools. We plan to spend time in the schools and neighborhoods to promote healthy lifestyles, and we will be measuring and recording outcomes. The VISTA will also work on volunteer recruitment for a pilot program we want to start, which provides supports for the NCHA care management team and possibly in the community health clinics. This program will help to connect people with resources to address their barriers to health. Additionally, the VISTA will help to develop an effective strategy map for the Thriving Weld Data Dashboard for the Community Care Corps work that we are doing. We will be able to populate the dashboard with measureable data and outcomes for our programs.
