CHPT - Community Clinic Resource Desk
 
 
 
Founded in 1999, Communities and Health Professionals Together (CHPT) began as an informal service-learning partnership between the UC Davis School of Medicine and several community based organizations. The program has quickly grown into a multifaceted and multidisciplinary collaboration; fostering medical outreach, child advocacy, and community service in young physicians. CHPT aims to improve child health outcomes throughout the Sacramento region by 1) increasing resident physicians’ understanding of the social determinants of health and 2) providing tools for engaging in reciprocal partnerships with local organizations via the lens of Asset Based Community Development. In February 2014, CHPT partnered with a local community clinic to create a patient resource desk. The purpose of the service is to help pediatricians identify families impacted by social determinants of health and connect those families with appropriate resources. The desk is staffed by undergraduate volunteers and operates under a case management model. Volunteers make referrals to local organizations, follow-up with families until need is met, and provide regular updates to the physician throughout the process. A VISTA member is needed to further develop the program and implement sustainability strategies.

Member Duties : The VISTA will build the program by: 1) Recruiting and train new volunteers. 2) Supporting volunteers by providing feedback, following their casework, and meeting with them regularly. 3) Building partnerships with local agencies. 4) Developing materials for volunteers and patients. 5) Planning for the sustainability of the volunteer program through the creation of a student leadership council. The VISTA will also support some of the other operations of the CHPT program, such as fund development, creating promotional materials, maintenance of social media, and planning workshops and events.
