Wellness Coordinator for Elder Services
 
 
 
The Wellness Coordinator for Elder Services will develop, implement and evaluate systems designed to increase access to a robust array of meaningful activities in order to support their health, wellness, and community engagement. These include preventative health screening services, group exercise activities, and wellness seminars, such as nutrition or gardening. Systems seek to increase access to affordable, healthy, and traditional foods for Elders, to facilitate the transfer of cultural practices for traditional foods from Elders to other Native community members, and to seek a comprehensive approach to the decrease of health issues (e.g. diabetes and obesity) to Elders of the Portland Metropolitan Area's Native American community. Member Objectives include: 1) Analyze the current calendar of activities provided to Elders with respect to the type and balance of activities available, the quality of communication about such activities to Elders, and performance of a needs assessment to identify areas with potentially unmet wellness needs (e.g. health screenings, group exercise classes, and wellness seminars). 2) Develop both a short-term plan (spanning the remainder of the service year) and long-term strategy for “wraparound” services plan for Elders that includes goals, specific strategies, and a timeline to improve the quality and integration of wellness service offerings (e.g. access to healthy food and housing stability) to ensure that the wellness needs of Native Elders are addressed in a holistic manner. 3) Develop an engagement plan for joint Youth & Elder activities in order to promote the passing on of cultural traditions and strengthen the overall sense of community in alignment with the agency’s vision to be “Elder guided”.
