WVCNCS - Veterans Community Outreach Team

Mountain Hospice based in Barbour County, WV but covers a 7 county region. The VISTA project will focus on establishing outreach to all veterans in the area, but especially those who are homeless or living in poverty. Veterans will receive information about VA benefits for end of life care through community education presentations. Working with the We Honor Veterans program, this VISTA will: • create materials and make presentations that raise awareness among community healthcare and other organizations about the unique end-of-life needs veterans may face and the benefits to which they may be entitled; • collaborate with community groups to offer veterans, those who work with veterans, and families of veterans supportive outreach events, honorary events, and special activities; • establish and recruit for a “Vet-to-Vet” volunteer program
