Program summary: The AmeriCorps members will serve as health care advocates stationed throughout the targeted Delta counties, connecting residents to prescription assistance and other benefit programs. Members will engage in health education/promotion and physical activity programs/interventions. In addition education sessions, workshops and one-on-one meetings will be held to help residents understand the details of the prescription assistance plans available to them. AmeriCorps members will assist clients in filing for refills and follow-up with clients regarding program adherence and maintenance. Members will also disseminate information on health insurance, access and benefits to residents throughout the targeted region. 1. Assist host site(s) with the expansion and/or enhancement of pharmacy assist program operations and other benefits programs. 2. Assist with patient prescription medication acquisition and adherence. 3. Coordinate and support other community-wide efforts relevant to prescription drug coverage and/or other benefit programs outreach, education and enrollment Summer Feeding Program, Youth Activities, Childhood obesity programs ect. 4. Recruit volunteers to assist with host sites volunteers program. 5. Conduct community workshops and forums for uninsured and under insured potential PAP/MAP clients. 6. Distribute program materials (flyers, brochures, etc.) to potential clients. 6. Participate in CNCS sponsored events, i.e. 9/11 Day, MLK Jr. Day, and AmeriCorps Week. 7. Recruit PAP Rx/MAP clients and other community members to serve as local advisory council members. 8. Disseminate information on health insurance, access and benefits.
