Camden Coalition of Healthcare Providers
 
 
 
Using data from the Camden HIE and the Camden Health Database, which were developed and are managed by the Coalition, the Coalition can identify and target patients who make disproportionate use of hospital and emergency department services as well as the patients who use hospital services for illnesses not requiring that level of care. The focus population for HealthCorps Navigators in Camden will be patients of the city’s two FQHCs, and patients served by other Camden primary care providers who have high in-patient hospital utilization, multiple chronic conditions, compounding socio-economic challenges and who need care coordination services. There are 3,000 to 5,000 patients in Camden in this highest needs group, who often are left to fall through the healthcare system’s many gaps. Navigators enable the Coalition and its provider members to reach more high needs patients than they would otherwise be able to reach. Many providers do not have the financial and management resources to attract, train and/or compensate staff or volunteers to work in the community with their most complex patients. In addition, Navigators provide in-the-community, hands-on coaching, mentoring and engaging patient activities that are often not reimbursable by Medicare or Medicaid.

Member Duties : Navigators will work closely with two Federally Qualified Health Centers (FQHC) and local primary care providers to improve care for socially and medically complex patients. Specifically, Navigators will work in the community to coach patients on how to effectively and efficiently access the healthcare system to self-manage their chronic conditions, including understanding the financial impact of healthcare decisions, developing and implement nutrition/exercise activities for children, teaching patients about available social supports, and training. 8-9 Navigators will serve directly with the Coalition as part of its outreach teams, 2 will be placed with CAMcare (FQHC), 1-2 will be placed with Project HOPE (FQHC), and 4 will be placed with Cooper University Medical System’s internal and family medicine clinics.
