Transitions Program
 
 
 
Helping people live their best lives even amid a life limiting illness. Transitions is a community service program designed to enhance the quality of life for adults, and their families facing a life-limiting illness. Services include emotional support, education and practical assistance. Based on specific needs, an individualized plan is formulated for clients and their families. There is no fee or charge for services provided to Transitions clients. Our goal is to help our client and their families find and utilize community resources, navigate through their healthcare system and have practical support along the way Transitions Program relies heavily on trained volunteers to reach out to our community, especially our veteran organizations. We offer in-home, coordinated services to adults who have a life limiting illness. The program enhances clients quality of life by providing information, resources and support to help clients plan for anticipated life changes and make informed decisions about their care. Services are provided by the Transitions Program Coordinator, AmeriCorp member, social worker, MSW students, and trained volunteers. We give special attention to our Veterans through the “We Honor Vets” program and actively recruit and train veteran volunteers to serve our veteran clients. This position is located in Yakima, Washington.

Member Duties : Health education: Motivational interviewing, health literacy and other tools. Provide Training to volunteers/staff to utilize these tools. Report Quarterly: a. Develop a survey(s) to document the effectiveness of these tools. b. Number of presentations/attendance/contacts made etc. Education to veterans groups: Increase awareness of need for veteran volunteers. Recruit both volunteers and client veterans. Report Quarterly: a. Number of events, meetings, trainings provided and attendance. b. Number of veterans who volunteer/times they volunteer. c. Number of veterans who receive services as a patient/client as well as the outcome. Case management: Access to community resources, practical health care services and emotional support that are needed currently or in the future. Clarify goals so that care decisions reflect what is most important to clients and their families. Transition volunteer trainings: Nurture volunteer relationships, volunteer recognition and volunteer newsletter.
