Ryan Chelsea Clinton-Eligibility Specialist
 
 
 
Community HealthCorps member will be serving at the Ryan/Chelsea-Clinton Community Health Center, in New York City. Community HealthCorps is an AmeriCorps program of the National Association of Community Health Centers, Inc, (NACHC®) that promotes health care for America's underserved, while developing tomorrow's health care workforce. Members will be serving a diverse population of patients from the communities surrounding Ryan/Chelsea-Clinton. The Ryan/Chelsea-Clinton Community Health Center and its Facilitated Enrollment Program is committed to eliminating health disparities and providing high quality outreach services to the medically underserved in the community. The Program promotes the Center’s services; enrolls qualifying patients into appropriate entitlement programs; provides brief educational sessions; telephones patients to remind them of their appointments. Member will sit with the DEC's and have own computer and phone.
