The mission of NorthPoint Health and Wellness Center is "Partnering for a Healthier Community". As an organization NorthPoint is particularly unique because of the co-location of a federally qualified community health center and a Human Services agency operating as an integrated entity providing health and human care services using well-defined service and delivery protocols. NorthPoint's integrated services development strategy recognizes the interdependencies of complex issues that individuals face, so services are provided using "whole person" approach while also looking at the impact that PSE's also have on the individual's health. The VISTA position is responsible for providing support to the NorthPoint Human Services and the NorthPoint Clinic teams focusing on efforts aimed at eliminating health disparities in North Minneapolis. This position will increase health awareness related to the importance of prevention and health promotion, as well as social and economic issues impacting families in a variety of ways, including outreach. Outreach activities may be conducted in neighborhoods, agencies, churches, park and recreation facilities and schools. Will identify adults and families in need of health and social services and connect them with NPHWC clinic or human services staff. Will research and develop best-practices group education programming related to a specific health area, nutrition or general wellness. This position also provides: research and program development, client support, outreach, client referrals, support the North Side Healthy Eating Project and strengthen relationships with NorthPoint’s academic partners.

Member Duties : •Provide assistance with health education, outreach, and referrals to social service at NPHWC and within the community •Recommend changes in existing health-related social service programs, policies and procedures. •Attend mandatory team meetings/trainings; seek out professional development opportunities. •Recruit and train volunteers as needed to support outreach and inreach program services. •Prepare and maintain records and reports related to outreach (ability to design work plans and timelines to accomplish expected goals). •Partner with other community organizations and participate in events/outreach to provide education for specific health areas, for the Eliminating Health Disparities Initiative and the Health Care Home model. •Support NPI general outreach efforts as assigned by supervisor and EHDI team. Interviews will occur on a rolling basis and preference will be given to candidates who apply before April 12. Candidates must also email a resume and cover letter.
