| Member Duties : Interview staff and residents across all programs to (1) identify services and resources available within each program and (2) assess outside services and resources needed by clients during their stay and on discharge, such as long term mental health care, higher education and childcare.
Identify partner agencies that can fill identified needs and establish relationships with said partners on behalf of the organization.
Consolidate resources to create a comprehensive guide to be distributed to all clients upon intake and discharge, with consideration for average literacy levels of served populations.
Audit existing linkage agreement documents.
Establish new and renewed community partnerships and facilitate partnership development through referrals and educational and/or promotional events.
Identify and implement database for maintaining linkage paperwork and contact information.
Create legacy document detailing standard operating procedures for linkage agreements.
