Healthier Boynton Beach Project Support Specialist
 
 
 
Healthier Together is a community-driven, multi-year, placed-based, funding approach developed by Palm Health Foundation. This strategic initiative convenes neighborhood leaders and residents, listens to their needs, and provides financial support and resources to help the neighborhood make positive, healthy changes. Healthier Boynton Beach is the third of the six communities funded by Palm Health to create equitable health outcomes in family caregiving. Healthier Boynton Beach promotes regular outreach/training programs and awareness campaigns, recognizes caregivers and conducts assessments to help caregivers self-identify and to increase awareness in the community. Increasing education, Healthier Boynton Beach advocates for the statewide adoption of the Caregiver Advise, Record, Enable (CARE) Act, identifies services, supports and resources and circulates information to caregivers. Sometimes, a caregiver’s own health gets overlooked. To improve health and wellness, Healthier Boynton Beach helps to identify, coordinate and connect health and wellness services to caregivers. Member Duties : In year two, the Healthier Boynton Beach Project Support Specialist will work to increase community awareness, community connections and community engagement while addressing the needs and issues of family caregivers in Boynton Beach by assessing current stakeholders’ level of engagement and increasing awareness of supportive programs. Healthier Boynton Beach engages residents who identify as ‘caregivers’ to strengthen their understanding and accessibility of resources. The Project Support Specialist will develop a "Healthier Boynton Beach Social Media Campaign" to connect family caregivers and community residents to opportunities and available resources; redesign the Family Caregiver Registry database to improve awareness and engagement activity; and design and develop a process to enhance the current Mini Grant process for Healthier Boynton Beach.
