HomeFront Health Access Coordination VISTA
 
 
 
This VISTA project is designed to improve access to health services for clients of HomeFront, a leading Central New Jersey nonprofit that offers emergency shelter and comprehensive services for homeless and very low income families. The project aims to formalize health access coordination throughout HomeFront locations by strengthening and developing new relationships with health care providers, developing a new system to maintain, track, and schedule health education and access activities, and advising HomeFront staff on areas of improvement as it relates to health and wellness community-wide.

Member Duties : -Strengthen and develop relationships with health care providers in the community through networking with existing providers and identifying obstacles to receiving health care -Develop database of health care providers and volunteers to be used as a resource for HomeFront staff and clients an an online calendar for health care opportunities -Advise HomeFront staff on areas of improvement as it relates to health and wellness including recommending training and in-service needs, developing health care protocols, and formalizing agency health access procedures
