This project will serve rural, low-income seniors to prevent food insecurity by formalizing a network of service providers including a referral and follow up system between hospital discharge planners, healthcare providers, and community-based organizations. The VISTA volunteer will establish a pilot project by selecting the geographic area, establishing a network of providers, build the follow up and referral protocols, and other items as needed. In addition, the VISTA volunteer will work with the NMAAA nutritionist to develop and present nutrition education materials and trainings for seniors, senior service providers, and health care providers in relation to this pilot project.
