NCNMEDD_Non-Metro Area Agency on Aging Senior NutritionVISTA
 
 
 
Non-Metro Area Agency on Aging (NMAAA) oversees comprehensive service delivery of community and home-based services in 32 of New Mexico’s 33 counties. Our goal is to assist seniors in the state of New Mexico with maintaining their independence and dignity. NMAAA is located in Santa Fe. This VISTA project will serve rural, low-income seniors to prevent food insecurity by formalizing a network of service providers including a referral and follow up system between hospital discharge planners, healthcare providers, and community-based organizations. The VISTA volunteer will establish a pilot project by selecting the geographic area, establishing a network of providers, build the follow up and referral protocols, and other items as needed. The VISTA volunteer will work closely with the NMAAA nutritionist, senior center providers, and other partners to launch this project. Work will include travel, networking, and professional development in related subject areas.

Member Duties : The VISTA volunteer will conduct a community assessment, outreach and planning. The volunteer will research existing data and conduct informational interviews to determine critical nutrition needs of rural, low-income, senior New Mexicans. The volunteer will collaborate with stakeholders to implement a nutrition plan for seniors upon hospital discharge and will build a network among healthcare providers and community organizations. The VISTA volunteer will develop a strategic plan for the project with vision, goals, objectives, and timelines. The volunteer will implement the pilot project and develop tools to measure the impact and outcomes of the project. The VISTA volunteer will also develop and present nutrition education materials and trainings for seniors, senior service providers, and health care providers in relation to this pilot project.
