Western Mountain Regional Health Alliance (UWBB)
The mission of West Mountain Regional Health Alliance (WMRHA) is to build an integrated healthcare system to achieve optimal health for all people who live in the West Mountain region. WMRHA aligns health providers and stakeholders and initiatives in the West Mountain region to build an integrated health system. They also expand access to a continuum of affordable, high-quality, integrated medical, behavioral, and oral health care for the medically under-served residents of the West Mountain region. WMRHA also supports affordable, integrated medical, behavioral, and oral health care in the West Mountain region’s health system. The VISTA member will support WMRHA by building the capacity of the current homeless response system. Housing is now a well-documented determinant of health. Ensuring all residents of the West Mountain Region have a safe, clean home is vital to improving health within the community. Over the course of the service year, the VISTA will collaborate with local partner agencies, facilitate meetings, and apply for funding to support the efforts. In addition, the VISTA will develop and gather relevant data from partners for strategy maps and scorecards using public health and healthcare shared measures as well as measurements of Social Determinants of Health. The VISTA member will also develop a workflow process and establish a manual and forms to ensure strategy maps and scorecards are kept up-to-date and accurate.

Member Duties : To help ensure that residents of West Mountain (Eagle, Garfield, and Pitkin counties) break the cycle of poverty and receive integrated health care and support around Social Determinants of Health. The VISTA will build upon the communities existing homeless response system by convening stakeholder meetings and analyzing data on the current homeless population especially those with complex needs. The VISTA will build the capacity of the West Mountain Regional Health Alliance by helping to develop and maintain a regional data dashboard that will serve WMRHA partners and the broader community. The VISTA will additionally support fundraising efforts to build resources that are driven by data identified gaps.
