3 Month UCA - Money Management Community Health Advocate
 
 
 
The Community Health Advocate will provide free application and referral assistance for low to moderate income individuals and families in need of health coverage in Douglas County. The Serving at United Community Action Network's Money Management Program in Roseburg, Oregon, the member will support clients completing health coverage applications, as well as provide wrap-around referrals to social service projects for which they qualify (i.e. energy assistance, weatherization, housing, food pantries, domestic violence advocacy, etc.). Additionally, the member will develop pathways for community volunteers to provide application and referral assistance at community outreach sites. The member will train, mentor, and support volunteers in being volunteer advocates that help clients navigate applications and referrals for Oregon Health Plan (OHP) other social services.

Member Duties : • Provide OHP assistance by supporting clients in applying for health coverage • Providing application assistance and referrals for other UCAN and community programs such as energy assistance, weatherization, rental assistance, housing, food pantries, HIV Alliance, and domestic violence supports. • Develop opportunities for volunteers to serve both short term and long-term positions as OHP assisters • Develop in-reach opportunities within UCAN programs to provide health care enrollment for clients of other agency services. • Provide outreach to partner agencies to include food pantries, the Douglas Educational Service District, FDS and other social service entities. • Implement and/or develop systems to inform Effective Volunteer Management Practices (EVMPs).
