Appalachia HealthCorps is working across two states with a variety of partners to implement models of chronic care coordination that includes Community Health Workers (CHW) as part of the care team for patients struggling to control their chronic conditions. We have seen dramatic improvements in health and equally dramatic reductions in A1C (a measure of diabetes severity) through the model and are now working with multiple insurers to determine how much the model saves in health care costs. The Washington County Commission is using Community Health improvement Associates to manage this AmeriCorps program which will give us enough “boots on the ground” to widely replicate the model.

Member Duties : Community Health Workers (CHWs) are remarkably successful at helping patients do what the doctor wants them to do. In this project, you will be a CHW and report to a nurse or a lead CHW. We will train you on Chronic Pain Management, Motivational Interviewing and other evidence-based programs, and you will visit patients we assign to you on a weekly basis. You will increase access to care for these patients and provide one-on-one health education services. AmeriCorps members do not provide health services related to prohibited items or activities, including abortion referrals or services, as will be outlined in detail during orientation. We have seen health miracles happen with this type of care. We are looking for AmeriCorps members in Wood and Randolph Counties in WV and Meigs, Athens and Ross Counties in OH.
