NYU Langone Sunset Park AmeriCorps - Check & Connect Mentor
 
 
 
Sunset Park AmeriCorps is a full-time, service program for people ages 18+. Must be a U.S. citizen or permanent resident. Members work directly with the community-based programs through the Family Health Centers at NYU Langone. The Check and Connect intervention model aims to increase students’ academic engagement through targeted mentoring of students who face challenges and need additional support for academic success. Check & Connect Mentors will use this evidence-based intervention model and work with a cohort of students throughout the year. The Check and Connect Mentor is responsible for supporting students’ academic, social-emotional, and personal development through consistent mentoring, progress monitoring, and family engagement. At the core of our program model is creating a trusting relationship between the student and a caring, trained mentor who both advocates for and challenges the student to keep education a priority. The mentor will build meaningful relationships with students on their caseload while collaborating with school staff, families, and program leadership to improve student attendance, academic performance, and overall student success. The position requires strong communication, organization, and documentation skills to ensure students receive targeted interventions and ongoing support. Members serving at our John Jay High School/Project TRACK program will be working with students grades 9-12.

Member Duties : • Weekly one-on-one check-ins with 10-20 students, providing academic and other assistance to students participating in afterschool programming using the Check and Connect Model • Weekly Monitoring Forms, gathering behavioral and school data. • Connecting students to needed interventions. • Connecting with families. Member support is provided through mentoring, academic support, service learning, and additional interventions. • Participate in case conferencing and other trainings and meetings as needed.
