6 Month UCA - AVIVA Health Patient Navigator
 
 
 
Serving in Roseburg, OR with Aviva Health, the AmeriCorps Patient Navigator will help guide patients through the complexities of the health care and social services system, under the direct supervision of the Social Determinants of Health Coordinator. The AmeriCorps will work closely with medical providers, primary care teams, and social services agencies to provide short-term care coordination and connection to resources and support to Aviva Health patients to improve their health and general well-being. United Communities AmeriCorps (UCA) members engage in meaningful service that meets critical community needs throughout Southwest Oregon. UCA projects improve access to social services by bridging barriers to educational opportunities, mentoring, job skills development, employment resources, family stability, and having healthy futures. Our members engage clients in their dedicated service project, and then use their experience to recruit, retain, and support community volunteers in their project and host site. Our positions are a dynamic combination of serving in a dedicated project (85%) and engaging in civic and professional development opportunities, alongside our AmeriCorps cohort (15%).

Member Duties : • Conduct outreach to identify and engage community members around health and wellness activities and social determinants of health. • Assist patients in accessing health-related and social-needs-related services. • Assess health status and social determinants of health. • Facilitate health education and behavior change strategies with individuals and groups. • Communicate with county and community-based social service agencies and professionals. • Provide patient care coordination and navigation • Collaborate with the Social Determinants of Health (SDOH) Coordinator to provide and address SDOH screenings. • Manage patient information, including entry into a computer-based system • Provide presentations to individuals and small groups. • Assist with the completion of patient forms/paperwork and scheduling appointments for health care and with social service agencies. • Provide advocacy to address health disparities at the individual and community level.
