JFCS Care Navigation VISTA
 
 
 
The mission of Jewish Family and Children's Service of Greater Philadelphia (JFCS) is to strengthen families and individuals across generations and cultures to achieve stability, independence, and community. JFCS is committed to continually adapting and expanding its services to meet the evolving needs of the greater Philadelphia community. The Care Navigation team serves as the front door to the organization, ensuring that every individual who contacts JFCS receives a timely, compassionate, and trauma-informed response. The AmeriCorps VISTA will play a key role in strengthening the infrastructure of the Care Navigation program by helping develop and expand community resource and referral networks, cultivating relationships with partner organizations, organizing and maintaining referral information, and identifying opportunities to improve coordination across the community. In collaboration with the Care Navigation and Data and Evaluation teams, the VISTA will also support data collection, reporting, process mapping, and evaluation activities to inform the development of a sustainable intake model. This position is part of National Nurse-Led Care Consortium's VISTA program.

Member Duties : 1. Strengthen JCFS care navigation systems by evaluating current intake operations and developing a sustainable staffing and workflow model that ensures timely, trauma-informed access to services. 2. Collect and analyze data on call volume, response times, referral patterns, client needs, and service trends to inform continuous quality improvement and long-term planning. 3. Expand community referral networks by researching community resources, strengthening relationships with partner organizations, and creating an up-to-date referral database that improves coordination of care and access to services. 4. Enhance equitable access to services by supporting the development of a coordinated, responsive, and trauma-informed entry point that connects individuals and families to the appropriate internal and external resources as efficiently as possible.
