CO Public Health Works _Communtiy Health Worker

Local Public Health Agencies work to improve community well-being by providing services that help prevent disease, illness, and injury, particularly in communities (populations) most adversely impacted by social, economic and environmental disparities. Community Health Workers are frontline public health connectors that have a close understanding of the community they serve. This trusting relationship enables them to serve as a liaison between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. Community Health Workers build individual and community capacity through activities like outreach, community education, informal counseling, social support and advocacy. This Program leverages support from AmeriCorps through Serve Colorado – the Governor’s Commission for Community Service to empower community-based organizations to meet locally identified needs and statewide challenges through service.
