St. Clare 2025 -2026 Operations/QI Associate

St. Clare Health Mission is a non-profit organization dedicated to improving community health outcomes. This AmeriCorps position will focus on building capacity to address the growing epidemic of Type 2 diabetes within our service area, particularly among underserved populations. The member will play a pivotal role in developing and implementing a Diabetic Collaborative Practice Agreement (CPA) and leading the clinic-wide transition to a new Electronic Health Record (EHR) system. The goal of this project is to create sustainable systems that will enhance patient care, streamline clinical workflows, and improve data collection for program evaluation and grant reporting. This is a capacity-building role, meaning the member will not provide direct patient care but will instead create and improve the systems and processes that enable the organization to better serve its community.
