Member Duties : Lead development and implementation of a Collaborative Practice Agreement (CPA) for chronic disease management, with emphasis on diabetes care. Collaborate with physicians, nurses, and pharmacists to draft/finalize the CPA, design staff training, and create systems to monitor compliance and outcomes. Serve as project manager for the clinic’s transition to a new EHR system (main and mobile clinics), coordinating with vendors to ensure smooth rollout, developing workflows/templates, and providing training and implementation support. Partner with leadership to define key performance indicators (KPIs), oversee data collection and analysis in the EHR, and prepare program evaluation reports for grants and compliance.
