Erie Family Social Drivers of Health Navigator (Evanston)
 
 
 
The SDOH Navigator also links Erie patients to resources within Erie and in the community. The SDOH Navigator enrolls patients in benefits, schedules medical appointments, and navigates social drivers of health resources such as rental and utility assistance, food assistance, medical transportation, as well as others. The SDOH Navigator will complete data entry and reporting requirements for SDOH, assist with SDOH patient registry, and assist with managing SDOH resources. This position is at our Evanston location, 1285 Hartrey Ave, Evanston, IL 60202.

Member Duties : Screen clients for needs around social drivers of health either via telephone or in-clinic and connect to internal and external resources. Provide linkage to services such as emergency financial assistance, transportation, food assistance, etc. Enroll patients in services, benefits, and programs for which they are eligible (pharmaceutical programs, LINK, financial applications for hospital referrals, early intervention support, DME, urgent referrals etc.). Assist patients in digital navigation, including MyChart enrollment, setting up telehealth capabilities, and other digital assistance related to health. Educate and empower patients to navigate future SDOH needs. Process internal referrals to the Patient Programs Department. Take on short-term agency initiatives related to SDOH, as they arise.
