Erie Family School Based Health Health Educator (Lakeview)
 
 
 
The Health Educator will provide students with vital information regarding their own health and how to advocate for themselves in the health care system. Through the education and outreach efforts of the health educator, students will learn about the health care facilities that Erie operates and their ability to access the centers without experiencing barriers. Often, the Health Educator is a student’s initial point of contact with Erie. This position is located at our Lakeview High School location, 4015 N Ashland Ave, Chicago, IL 60613.

Member Duties : Coordinate 1:1 nutrition/health coaching meeting with students. Implement health education curriculums to students in schools and community organizations. Arrange for Vision Specialists to see students at school. Arrange for Advocate Dental Van to see students. Develop monthly adolescent health topics to address on health center bulletin boards and newsletters. Host health promotions activities in the lunchroom and at school events. Support promotion, recruitment and engagement of Erie’s School Based Advisory Board and the Youth Advisory Committee. Facilitate projects in collaboration with school-based health leadership and staff to decrease barriers and increase engagement in health center initiatives.
