Assesses patients’ educational needs and barriers. Performs brief health education interventions using approved curriculum, and in consultation with the care team. Conducts a minimum of 85- 1:1 health education visit per month. Responsible for promoting group programming including identifying appropriate patients and recruiting a minimum of 10 patients per month for groups. Conducts outreach and follow-up when needed, schedules follow-up medical appointments when necessary, and refers patients to appropriate patient program services, including care management and social drivers of health. Collaborates with other members of patient programs department. Participates in approved community outreach and education events. Through outreach and visits support productivity and health outcome targets for department wide disease management activities. Supports with recruitment, coordinates logistics, and when needed supports with facilitation of Diabetes Prevention Program. This position is located at our Humboldt Park location, 2750 W North Ave, Chicago, IL 60647.

Member Duties : Health educators promote healthy lifestyle modification and deliver self-management education to at-risk and high-risk patient populations via in-person and/or telehealth visits. Health Educator delivers 1:1 health education visit, facilitates group sessions, completes disease management assessments, conducts outreach, and schedules appointment. This position requires someone with deep commitment to providing compassionate care that honors and individuality of every patient. An ideal candidate enjoys patient interaction and collaborating with program leaders to coordinate services/programs, have strong communication skills to effective communicate with patients, health care providers, and team members and enjoys working as part of a collaborative team to achieve high levels of productivity and maximize services, flexibility to adapt to change, and ability to independently handle patient care responsibilities and other work as assigned.
