Member Duties : Service Description : PRIMARY RESPONSIBILITIES/ESSENTIAL FUNCTIONS:
• Assist individuals in navigating health, social, and economic resources
• Assist patients with MyChart enrollment
• Conduct reminder, wellness and follow-up phone calls
• Connect patients and community members to Primary Care Physicians (PCP)
• Schedule patients for transportation
• Enroll patients in health insurance benefits
• Educate patients on chronic health diseases and tobacco cessation
• Conduct blood pressure checks
• Serves as member of the Sinai Care Cart
• Other duties as assigned based on service position/location
