Sinai AmeriCorps Initiative
 
 
 
The Sinai AmeriCorps Initiative offers members an opportunity to make a real difference in healthcare and in the lives of people including your own. Corps members tackle issues related to patient navigation, health access and wellness. Members assist individuals in navigating health, social, and economic resources; identify barriers to healthcare and gaps in healthcare services; provide health education and community outreach; provide system navigation, referrals, and linkage to care; and conduct research and evaluation activities including data collection and analysis.

Member Duties : Service Description : PRIMARY RESPONSIBILITIES/ESSENTIAL FUNCTIONS: • Assist individuals in navigating health, social, and economic resources • Assist patients with MyChart enrollment • Conduct reminder, wellness and follow-up phone calls • Connect patients and community members to Primary Care Physicians (PCP) • Schedule patients for transportation • Enroll patients in health insurance benefits • Educate patients on chronic health diseases and tobacco cessation • Conduct blood pressure checks • Serves as member of the Sinai Care Cart • Other duties as assigned based on service position/location
