The member will help bridge the gap between health care providers and patients by helping patients navigate the healthcare system, community resources and social support services by establishing trusting working relationships with patients and their families while providing general support and encouragement. This position is unique as the only community health worker in the system. In this position, the CHW will work closely with a team of Community Health Educators to provide outreach and support to families needing help navigating the health care system.

Member Duties : As an extension of the Laurel Health Center healthcare team, community health workers (CHW) assist in coordination of care and education, identifying individuals and families to assist in healthcare utilization. CHW’s contribute to improved health outcomes through patient outreach to enhance continuity of care and provide support in treatment plans for preventative care and disease management. CHW’s advocate, educate, and support patients in order to help them improve their lifestyle and connect them with appropriate healthcare options.
