Member Duties : As an extension of the Laurel Health Center healthcare team, community health workers (CHW) assist in coordination of care and education, identifying individuals and families to assist in healthcare utilization. CHW’s contribute to improved health outcomes through patient outreach to enhance continuity of care and provide support in treatment plans for preventative care and disease management. CHW’s advocate, educate, and support patients in order to help them improve their lifestyle and connect them with appropriate healthcare options.
