The Population Health Community Health Worker (CHW) position will fill several critical unmet needs within the Wright Center's community and patient population. These needs often fall outside the traditional scope of clinical care but significantly impact health outcomes and well-being. Clinical staff (doctors, nurses) primarily address medical needs, leaving a gap in addressing factors that influence health outcomes and integrated care support. The CHW uniquely fills this void by embedding these crucial services directly within the Wright Center's primary care model, tailored to our patients. This provides a complementary resource navigation function, often extending beyond the scope of traditional clinic-based social work to include broader community health initiatives

Member Duties : The Community Health Worker (CHW) works closely with medical providers, primary care teams, and social services agencies to provide short-term care coordination. The CHW will create connections to resources and support to program patients to improve their health and general well-being through education and provision of coordination of care and services. Targeted patient outreach will be required to ensure the appropriate care gaps are addressed and closed in a timely manner.
