SCFCA Community Health Workers Program PY 24-25 (JULY)
 
 
 
The South Carolina Free Clinic Association (SCFCA) proposes to have AmeriCorps members who will serve as Community Health Workers (CHWs) to provide health and social services navigation in free clinics in the following counties: Aiken, Anderson, Beaufort, Charleston, Georgetown, Greenville, Greenwood, Jasper, Kershaw, Laurens, and Pickens. Member Duties: Each AmeriCorps member will be assigned a caseload of clients. AmeriCorps members will be responsible for building an initial patient profile, conducting telephonic and in-person outreach efforts, health coaching, and creating a community resources guide for the clinic.
