Homeless/Healthcare Outreach Specialist
 
 
 
The Catholic Charities SWLA (CCSWLA) Homeless/Healthcare Outreach Program works to reach people who might not otherwise seek assistance or come to the attention of the homelessness service system while ensuring that people’s basic needs are met while supporting them along pathways toward housing stability. The Homeless/Healthcare Outreach Specialist conducts street outreach to engage with individuals experiencing homelessness who may be disconnected from mainstream services and support. Our street outreach efforts are person-centered, trauma-informed and culturally responsive. CCSWLA serves as an access point for individuals/families to the Coordinated Entry System, which are based off HUD’s guidelines and are Housing First oriented. Clients are also connected to supportive services such as access to temporary or permanent housing, SNAP, Medicaid, medical and behavioral health services, social security, identification cards, birth certificates, access to food, water, clothing, shower and laundry services. CCSWLA can also provide mediation services between family members, hosts and/or landlords along with assistance with identifying natural supports that could provide alternative housing and coordination of transportation services.

Member Duties : This position is a part of Catholic Charities USA’s Healthcare Outreach Program, a national intermediary project. As a member, you will meet virtually with other members serving throughout the country, all focused on assisting individuals within healthcare outreach. The AmeriCorps member will conduct street outreach, build relationships with and connect unhoused individuals to Coordinated Entry, and assist individuals with access to basic needs, emergency shelter, housing, healthcare, mental health treatment, Medicaid, SNAP, food and other services. Members will assist in transporting, setting up, and facilitating the mobile laundry/shower trailer and data collection. Members will be expected to collaborate across sectors to leverage resources, screen for Social Determinants of Health, and consider the client population when tailoring strategies for providing support services.
