PRIMARY RESPONSIBILITIES/ESSENTIAL FUNCTIONS: • Work with patients to assist with Medicare and Medicaid redetermination and Benefit enrollment check • Assist with data requests; database development; and data entry, cleaning, and coding. • Assist in the administration of surveys, questionnaires, and focus groups. • Analyze and secure outcome data. • Assist with evaluations. • Conduct health outreach and education. • Assist individuals in navigating health, social, and economic resources. • Respond to needs regarding disease outbreaks. • Research trends in healthcare to support administrative team. • Present research findings to internal audiences. • Assess available data to identify gaps in services and present recommendations for impactful change. • Identify opportunities to address barriers to accessing healthcare. • Facilitate community education presentations about healthcare in the community
