shares responsibility for providing health care for the team’s patients in a clinic setting. Accepts ownership and responds effectively to patient, team or personal concerns and works towards positive resolution. Engages in an environment of continuous learning, embracing change and mastering competencies of new skills. Assists in the orientation of individuals social assistance navigation especially insurance support. Review with patients and provide resources for them to get further assistance. Keep current on resources for patients and attend meetings. Provide patient with information or referrals needed for additional care. Knowledge in community resources, collaborations and other health care organizations and providers,
referral options. Understands and is knowledgeable with Clinic Policy and Procedures.
