Public Health AmeriCorps - Housing Transition Program
 
 
 
Responsible for assisting individuals residing in nursing facility care to prepare for and implement steps to transition to permanent supportive housing in the community. Provides case management interventions that facilitate illness self-management, skill building, identification of natural supports, and use of community resources. Serves as a liaison with other systems of care. Documents all services using program standards. Provides community-based case management services to individuals post-transition. Works with team members to implement programming based on participant-driven needs and ideas. Participates in all required Public Health AmeriCorps initiatives.

Member Duties : 1. Uses intensive outreach strategies to engage participants in services. 2. Ensures that participants have access to needed basic resources (housing, clothing and food) and medical, dental, mental health, substance abuse, and social services, governmental and public entitlements. Links participants to these community resources pre or post-discharge from nursing facility timely and according to participants’ needs. Assists transition team in transition logistics. 3. Teaches participants how to utilize community resources and promotes healthy socialization and recreational opportunities. Provides skill building activities in the community to assist participants in increasing their overall functioning as determined by participants’ needs and supervisor’s directions. 4. Educates participants about mental health, substances, medication self-management, healthy living and wellness, and independent living skills. 5. Documents services in a timely manner
