Member Duties : 1. Uses intensive outreach strategies to engage participants in services.
2. Ensures that participants have access to needed basic resources (housing, clothing and food) and medical, dental, mental health, substance abuse, and social services, governmental and public entitlements. Links participants to these community resources pre or post-discharge from nursing facility timely and according to participants’ needs. Assists transition team in transition logistics.
3. Teaches participants how to utilize community resources and promotes healthy socialization and recreational opportunities. Provides skill building activities in the community to assist participants in increasing their overall functioning as determined by participants’ needs and supervisor’s directions.
4. Educates participants about mental health, substances, medication self-management, healthy living and wellness, and independent living skills.
5. Documents services in a timely manner
