2024-25 Healthcare Outreach Coordinator
 
 
 
The Healthcare Outreach AmeriCorps Member would be a vital part of connecting refugees to the healthcare systems in their communities. The Member would assist clients with training on how to schedule primary care and specialist appointments. The member will accompany clients on appointments to provide navigation and coaching assistance. The Healthcare Outreach AmeriCorps Member will develop education health orientations with clients to promote self-sufficiency. The AmeriCorps Member will conduct outreach services to gain awareness of health clinics that provide services to the refugee community and those clinics that provide services on a sliding scale. The member will then compile these community resources and share them with clients. In addition, the AmeriCorps Member will look at resources for low-cost prescriptions and insulin programs.

Member Duties : The AmeriCorps Member will work in-person 4 days per week and virtually 1 day per week at Catholic Charities of Atlanta Northlake office in DeKalb County. The AmeriCorps Member will: complete outreach duties including becoming familiar with community resources related to healthcare services; collaborate and build relationships with community stakeholders; assist clients with scheduling primary care and specialist appointments; accompany clients in-person to healthcare appointments to provide assistance with navigation; assist clients with how to get prescriptions filled at local pharmacies; assist clients with identifying low-cost prescriptions and insulin programs; and provide healthcare orientation in the US workshop.
