Eco3.23 Wilderness Health Population Health Coordinator

Wilderness Health is a collaborative of independent providers working together to improve health care in Northeast Minnesota and Northwest Wisconsin. We believe there is value in keeping health care local. Creating sustainable health care models that aim to low costs and increase quality of care helps us keep health care in our communities. Through smart strategies, such as shared services and accountable care organizations (ACOs), Wilderness Health will lead the transformation for rural health care systems. Throughout their service our VISTA will help our network members’ patient populations attain optimal health and lead fuller lives. Our VISTA will assists with coordination of cross-network population health strategies and activities and take an active role in developing and executing health promotion efforts, working with community organizations, and participating in network and community-organized health improvement initiatives. A major component of their position with be assisting with the development, implementation, and maintenance of our telehealth program including development of policies and procedures, data collection and reporting. The VISTA member will be part of the Ecolibrium3 AmeriCorps VISTA cohort serving in northeastern Minnesota. Although each member is serving in a unique position, all cohort members will participate in VISTA specific activities such as regular cohort meetings, collaborative projects, professional development opportunities, and trainings.

Member Duties : 1) Assist with the development, implementation, and maintenance of the telehealth program including development of policies and procedures, data collection and reporting. 2) Support and coordinate activities that address the network strategy for integrative behavioral health. 3) Develop programming and provide health and digital literacy education to support patient access and comprehension of health information. 4) Develop marketing and educational materials and resources for outreach. 5) Research, collect data, and support evidence-based strategies to address mental/behavioral health. 6) Assist with data aggregation, collection procedures and tracking of project metrics.
