Member Duties : • Community Engagement - Meeting with Dane County Social Workers to help identify community residents lacking healthcare and in need of a medical home.
• Supporting the Perry Family Free Clinic (PFFC) Physicians by doing patient check-ins on clinic days.
• Supporting UW Madison School of Pharmacy Students - Operation Diabetes program.
• Helping PFFC patients complete Health History Questionnaires.
• Phone call follow-up with patient appointment reminders.
• Occasionally picking up and dropping off PFFC patients with agency vehicles.
