Access and Community Engagement VISTA (McMinnville)
 
 
 
This position joins the Oregon Health Authority’s statewide AmeriCorps VISTA team. In addition to activities at your individual site, each member receives additional orientation, training throughout the year, team retreats and meetings, and the support of the team leaders and project director based at the state public health agency. Yamhill Community Care (YCCO) is one of 16 coordinated care organizations (CCO) in Oregon that manage care and provide resources for members on the Oregon Health Plan (OHP), the state's Medicaid system, which serves those experiencing poverty in Oregon. OHP is a unique Medicaid model which assigns Medicaid members to a CCO, giving them access to a designated primary care provider, services to manage their health, and other flexible benefits designed to address the whole person, including social needs, location, and identities. YCCO’s governing committee, the Community Advisory Council, is composed of YCCO members and family of members. YCCO is advised by other committees made up of local stakeholders to inform its decisions. YCCO has its own Early Learning Hub, which focuses on strengthening families, coordinating and aligning services, and improving school readiness. The Member will work with YCCO, its various committees, as well as community-based organizations and county services to help coordinate the best care possible for all members, as well as create and monitor programs that address poverty and its effects on health outcomes. YCCO offices are in McMinnville, Oregon. A smaller town in the heart of wine country, home to Linfield College, McMinnville is an hour from Portland, an hour from the coast, three hours from Mt. Hood, and surrounded by gorgeous hiking and sightseeing opportunities.

Member Duties : 3 areas of focus: Social Determinants of Health: 1) Support convening community members and orgs to understand needs. 2) Develop multilingual presentations and educational materials to share with community. 3) Evaluate engagement efforts and build improvement plans. Community Improvement Plan: 1) Evaluate current health improvement plans and projects. 2) Convene community groups using the Mobilizing for Action through Planning and Partnerships Process to inform health improvement strategy development. 3) Create a 3-year Community Health Improvement plan for the region. Language Access: 1) Collaborate with language service providers and local clinics to map workflows for providing language services. 2) Coordinate culturally responsive care training for community agencies. 3) Review data to identify improvement needs and create a plan to support individuals with accessibility needs. Bilingual and individuals with experience in multicultural settings strongly encouraged to apply.
