Ronald McDonald House New York - Improving Community Health
 
 
 
At Ronald McDonald House New York, AmeriCorps members will advance health equity outcomes for underserved medically complex children and their families residing in New York City. AmeriCorps members will do this by performing outreach to families in marginalized communities and offer risk assessments to identify social determinants of health. Then members will refer families to community-based organizations and services at other nonprofits to address those social determinants of health. For example, if a family expresses food insecurity we will connect them to food banks and other sources. Members will also help families identify their primary care provider and set up appointments. Members may perform home visits where relevant, such as visiting a family to deliver essential baby items or help point out ways to make their apartment less susceptible to Asthma-inducing dust, etc. We receive lists of families with children that are either have illnesses or have not seen a primary care provider in a long time, and we call those families to encourage doctor visits, set appointments, conduct assessments for social determinants of health, offer information, connect families to additional resources and encourage better health. Occasionally, members will perform community assessments by visiting communities to identify broader community needs such as the lack of healthy food options in an area, etc. By participating, AmeriCorps members are not only helping families with children at-risk of illnesses, but also 1) build the knowledge surrounding what services help families with sick children, and 2) increase data available to demonstrate how social determinants of health impact families' access to critical care. Members will be assigned to families to follow up with them over the course of their program. And take detailed notes in our databases to allow for that care to continue even when the member completes the program.
