Community Health Pathways
 
 
 
The Community Health Pathways program was created in response to COVID-19, which further exacerbated historic inequities in racial health outcomes while also placing tremendous stress on individual and community-level support systems. Community Health Pathways is an 11-month service opportunity to learn and serve as a full-time AmeriCorps member. You will learn about healthcare and racial justice while serving with a Baltimore-based healthcare organization. Members will build trusted relationships with community members and provide services through various activities like outreach, follow-up, peer support, and resource referral. Members can gain experience in many community health services like violence prevention, mental health, and more. Participants will serve with local Baltimore community health organizations, including non-profit organizations, hospitals, and grassroots organizations. If you're looking for a pathway into healthcare and have little or no experience, this is the opportunity for you. At the completion of the program, CHP AmeriCorps members will earn professional licenses in the field (such as a Certified Nursing Assistant or Community Health Worker) and an education award. Participants will also join Baltimore Corps’ network of 10,000+ Baltimore changemakers launching you into a healthcare career at top hospital employers and community health organizations.

Member Duties : Members can serve in two different capacities: Community Health Assistant - Serve to increase access to high-quality care for Baltimore’s underserved communities. Provide support to patients as needed, such as setting up appointments, gathering health information, or calling to check on people. Community Health Assistants may serve in medical facilities or in community clinics. Receive certification as a CHW. Violence Prevention Coordinator - Serve at a Baltimore-based organization to support staff and community members in implementing trauma-informed care strategies, provide safety planning and follow-up services to trauma or violence-impacted individuals, and guide affected community members to wraparound services. Violence Prevention Coordinators may travel throughout the City to support people and organizations. Receive certification as a CHW.
