VOA-Midstates Determined Health AmeriCorps
 
 
 
Determined Health employs Community Health Workers that educate and engage residents of Louisville's West End neighborhoods. They provide grassroots outreach designed to address the root causes of health disparities and inequities by focusing on the social determinants of health and connecting area residents with access to high-quality health care. The innovative new program employs trained CHWs who engage with the residents at increased risk of poor health by providing care coordination, offering support for chronic disease management, navigation of community resources, empowering individuals to make health decisions, community outreach, and increased health education. CHWs will coordinate health care services with health care teams to achieve successful health outcomes for participants. VOA goals for the program include reducing health disparities in Louisville, bridging the gap in care, and improving health outcomes for all program participants.

Member Duties : Gain valuable professional experience or help an organization expand its capacity, while making an impact in your community. Volunteers of America Mid-State is looking for 5 Full-Time AmeriCorps member(s) with a passion for public health to serve on our team. You’ll support with improving health equity, by: - Act as a liaison between clients, health care system and human services organizations. - Provide coordination of care - A trusted member of the community - Offer support for chronic disease self-management - Apply motivational interviewing skills - Provide health education - Link clients to healthcare providers - Familiar with chronic diseases such as diabetes, hypertension, etc. - Be trained as a Community Health Worker - Navigate clients to community resources.
