Health Share Social Determinants of Health VISTA (Portland)
 
 
 
This position joins the Oregon Health Authority’s statewide AmeriCorps VISTA team. In addition to activities at your individual site, each member receives additional orientation, training throughout the year, team retreats and meetings, and the support of the team leaders and project director based at the state public health agency. Selections are made on a rolling basis and early applications are strongly encouraged. The last day to submit an application is Sunday, July 31, 2022. Health Share of Oregon serves approximately 400,000 Oregon Health Plan members in Clackamas, Multnomah, and Washington counties. We bring together health plans, providers, and community health resources so our members can access the care they need to be well. The Oregon Health Plan (OHP) is our state’s Medicaid program. It provides no-cost health coverage—including medical, dental, and mental health and substance use benefits—to individuals and families who qualify. In addition to coordinating care and resources for members, we work with community partners in health care, education, housing, transportation, and social services to broaden health access and opportunity. Together with our partners, we are creating community-based solutions that lower everyday barriers to health, like providing rides to health care appointments, offering opportunities for peer support, and more. The OHA AmeriCorps VISTA Partnership Project is committed to building a diverse workforce to strengthen our work in Oregon communities. Member Duties : The AmeriCorps member will support several projects that aim to address Health Share members’ health-related social needs by expanding access to flexible services for vulnerable populations. The member’s activities will focus on: 1) building consensus regarding increased access to flexible services, 2) identifying 3-5 populations who whose social needs are impacting their health, and 3) increasing our community’s capacity to connect these individuals and families with flexible services and resources through streamlined processes and technological solutions. To be successful in this position, Health Share is seeking a candidate who demonstrates cooperation and teamwork, has excellent interpersonal skills and the ability to work successfully with a variety of stakeholders, and is a proactive problem-solver committed to tackling complex system issues to address the needs of low-income community members. The ability to clearly communicate in writing will be an asset in this work.
