- Screening for Structural Determinants of Health (SDOH) for new and current patients/clients
- Refer individuals to appropriate social services and healthcare services, as related to SDOH screening
- Provide healthcare education to high-risk individuals
- Take vital signs and blood pressure checks of patients/clients
